If desired, you can also assign a trusted family member fill out intake forms, update contact information, book appointments, and receive copies of email notifications.
Payment is due at time of service – Cash, Check, Venmo is preferred. HSA/FSA is accepted. All major credit cards except Amex is acceptable . A card service fee of approximately 3% is added to the total. All bodywork treatments in MN are subject to sales tax unless a medical referral from a MD or Chiropractor is provided
